WE SUPPORT: Person-Centred Supported Living in Trafford

WE SUPPORT LIMITED provides high-quality, person-centred supported living services for adults across Trafford.

We support adults with learning disabilities, autism, complex needs and behaviours that may challenge, providing flexible packages of support designed around each person’s individual needs, strengths, aspirations and choices.

Our approach is simple: people should have as much choice and control over their lives as possible.

Supported living enables people to live within their own home and local community while receiving the level of support they need to live safely, independently and meaningfully.

At WE SUPPORT, the person is at the centre of every decision. Where appropriate and with the individual’s agreement, we work closely with families, advocates, social workers, health professionals and other people who are important in their life.

Supported Living That Puts the Individual First

We believe that having your own home should mean exactly that.

People receiving supported living services should be supported to make genuine choices about:

  • where they live;
  • who they live with;
  • how their home is organised;
  • how they spend their time;
  • the activities they participate in;
  • the relationships that are important to them;
  • their education, employment and volunteering ambitions; and
  • the support they receive.

Our role is not simply to provide care. It is to help people build the skills, confidence and opportunities they need to live the life they choose.

Support is based around an individual assessment and agreed support plan, with a focus on promoting independence, rights, dignity, choice and inclusion.

Flexible Support to Meet Different Needs

There is no single model of supported living that works for everyone.

WE SUPPORT can provide a range of flexible support arrangements depending upon an individual’s assessed needs and commissioned support package.

Shared Supported Living

An individual may choose to live as a tenant in a shared home with other people who have compatible needs, preferences and lifestyles.

Shared supported living can enable people to benefit from their own tenancy and personalised support while sharing elements of staffing where appropriate.

We recognise that compatibility is extremely important. Individuals should be fully involved in decisions about where they live and who they share their home with.

Individual Supported Living

Some people require or prefer their own self-contained accommodation.

WE SUPPORT can provide personalised support within an individual’s own home, including packages for people with more complex needs who may require significant levels of dedicated support.

Where appropriate, we can work alongside the individual, their family, commissioners and housing providers to help identify suitable accommodation.

Outreach and Community Support

Not everybody needs 24-hour support.

We can provide flexible outreach support to people living independently or with family members, ranging from a small number of scheduled support hours through to more substantial packages.

Support can focus on developing independence, maintaining health and wellbeing, accessing the community, developing daily living skills and achieving personal goals.

Supporting People with Complex Needs

WE SUPPORT has experience supporting adults whose needs require carefully planned and consistent support.

This includes people with:

  • learning disabilities;
  • autism;
  • complex support needs;
  • communication difficulties;
  • behaviours that may challenge;
  • physical health needs;
  • mental health needs;
  • sensory needs; and
  • individuals requiring higher levels of staffing, including bespoke 1:1 and 2:1 support arrangements where commissioned.

We recognise that behaviour is often a form of communication. Our focus is on understanding the individual, identifying what is important to them, recognising potential triggers and reducing unnecessary restrictions.

Where required, support is delivered through structured risk management, consistent support approaches and positive, proactive strategies designed around the individual.

Supporting Independence and Everyday Life

Our supported living services are designed to help people develop and maintain the skills needed to live as independently as possible.

Depending upon individual needs, this may include support with:

  • Personal care and daily living
  • Medication administration and monitoring
  • Cooking and meal preparation
  • Shopping and household tasks
  • Managing money and budgeting
  • Accessing benefits and other entitlements
  • Maintaining a safe and comfortable home
  • Health appointments and healthcare
  • Emotional wellbeing
  • Community access
  • Maintaining relationships with family and friends
  • Developing friendships and social networks
  • Education and further learning
  • Employment and volunteering
  • Physical activity and healthy lifestyles
  • Hobbies, interests and meaningful activities
  • Travel training and developing independence
  • Building confidence and everyday life skills

These are only examples. Every support package is developed around the individual rather than expecting the individual to fit into an existing service model.

Finding the Right Home

Good supported living starts with the right home.

WE SUPPORT has experience working with housing providers and landlords across the North West to help identify accommodation suitable for people with learning disabilities, autism and complex needs.

Where accommodation needs to be identified, we can work alongside the individual, their family or representative, commissioners and housing providers to explore suitable options.

This can include shared accommodation as well as self-contained properties for people who have been assessed as requiring their own environment.

Importantly, care and housing should support an individual’s independence and tenancy rights. People should feel that the property is their home, not simply somewhere they receive a service.

Working with Families and Professionals

Families often hold invaluable knowledge about the person they support.

Where the individual wishes their family to be involved, we actively encourage positive family involvement when developing and reviewing support.

We also work collaboratively with:

  • Trafford Council social care teams;
  • commissioners;
  • social workers;
  • NHS and community health professionals;
  • advocates;
  • housing providers;
  • occupational therapists and other professionals; and
  • the individual’s wider support network.

This partnership approach helps ensure that support remains coordinated, responsive and centred around what matters to the individual.

A Focus on Outcomes, Not Just Hours of Support

Successful supported living is about much more than delivering commissioned hours.

We want to see people develop, achieve and participate.

Individual outcomes might include learning to prepare a meal independently, travelling somewhere without staff support, developing friendships, finding voluntary or paid employment, improving physical health, learning to manage money or becoming more confident within the local community.

For someone with more complex needs, progress may look very different. It could involve improved communication, greater tolerance of change, reduced distress, safer community access or gradually increasing choice and independence.

What matters is that outcomes are meaningful to the individual.

Supported Living Across Trafford

WE SUPPORT can develop and deliver supported living and outreach packages across the Trafford area, providing flexible support designed around the needs of each individual.

Whether someone requires a few hours of support each week, a shared supported living arrangement or a complex package involving substantial dedicated staffing, our aim remains the same:

to support people to have greater choice, control, independence and opportunity in their lives.

WE SUPPORT THE WAY YOU WANT TO BE SUPPORTED

We believe people with learning disabilities, autism and complex needs should have the same opportunities as everybody else to have a home, make choices, build relationships, participate in their community and plan for their future.

Our job is to provide the right support around the person to help make that possible.
